If you do not have a social security number, you must submit a signed statement including the phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct: I have never been issued a Social Security Number by the Social Security Administration.” You may fill this form out at the U.S. Embassy or Consulate.  It must be signed in front of the U.S. consul.  For more information, please see Frequently Asked Questions.
