Change or Correct Passport

Note:

  • Passports can no longer be amended; therefore you will need to apply for a new passport.
  • You must provide us with the original documents and their photocopies (single-sided). Original documents will be returned to you after reviewing your application.

Change/correction is within one year after issuance

If the change/correction is within one year after issuance of your passport, you may apply by mail (for both adults and minors age 16 and under).

Please send the following items to the Consular office in your district:

  • Valid passport
  • Form DS-5504 
    Complete online, print out single-sided, and sign. If applying for children under 16, a parent should sign the application form on behalf of the child.
    *NOTE: You must provide a Social Security number, if you have been issued one. If you do not have a Social Security number, please click here.
  • For corrections: Evidence of correct biographic information (original + 1 photocopy)*
    e.g. Original birth certificate, previous passport
  • For name changes: Legal documentation of name change (original + 1 photocopy)*
    e.g. Original marriage certificate, divorce decree, court order
    *All foreign documents must be accompanied by an English translation (formal or informal).
  • One passport photo
    White background / 2″×2″ in size/ no eyeglasses /no uniforms
  • Completed courier form
  • Passport fee

Change/correction is more than one year after issuance

If the change is more than one year after issuance, please follow the procedures for passport renewal and include:

  • For corrections: Evidence of correct biographic information (original + 1 photocopy)*
    e.g. Original birth certificate, previous passport
  • For name changes: Legal documentation of name change (original + 1 photocopy)*
    e.g. Original marriage certificate, divorce decree, court order

*All foreign documents must be accompanied by an English translation
