- A scanned copy of DS-3053 will not be accepted unless there is an emergency need to travel – If one parent/guardian cannot go with the child to apply for the passport, they can give permission by completing Form DS-3053 ^Statement of Consent.” You must submit the original completed consent form with the child’s passport application. The parent that cannot go with the child, also known as the non-applying parent, must:
- Fill out Form DS-3053 Sections 1-4, including minor’s name, minor’s date of birth, authorization validity for passport book and/or card, and statement of consent,
- Appear in person before a certified notary public,
- Sign and date Form DS-3053 only when instructed to do so by the certified notary public,
- Make sure the notary public completes the entirety of Section 5 for the Statement of Consent Notarization, and
- Submit a clear photocopy of the front and back side of the same valid government-issued ID that they present to the notary public with Form DS-3053.
- Note that DS-3053 is valid for only ninety days from the time of notarization.
- Please note: U.S. embassies and consulates provide notarial services, and the Form DS-3053 Statement of Consent can be notarized free of charge at those locations. In certain countries, a DS-3053 must be notarized at a U.S. embassy or consulate and cannot be notarized by a local notary public. Currently, these countries are Algeria, Angola, Bangladesh, Bulgaria, Cambodia, Cameroon, Dominican Republic, Egypt, Equatorial Guinea, Ethiopia, Gabon, Guatemala, Guinea, Haiti, Honduras, Iran, Iraq, Jamaica, Kenya, Laos, Lebanon, Libya, Mali, Mauritania, Mauritius, Moldova, Nicaragua, Nigeria, North Korea, Pakistan, Panama, Saudi Arabia, Senegal, Sierra Leone, Somalia, Sudan, Syria, Uganda, Ukraine, United Arab Emirates, Venezuela, Vietnam, and Yemen.
- The consular officer reserves the right to require additional evidence if the consent is notarized by a non-U.S. notary public.
- If you are the applying parent or guardian and the written consent of the non-applying parent or guardian or sole custody cannot be obtained in a timely manner complete item 4a-5 on the DS-5525 “Statement of Exigent/Special Family Circumstances” (PDF). The statement must explain in detail the non-applying parent’s or guardian’s unavailability and recent efforts made to contact the non-applying parent. You may also be required to provide evidence (e.g., custody order, incarceration order, restraining order) to document your claim of exigent or special circumstances.
